FAQ - Acupuncture and Dry Needling FAQs for Pain, Pelvic Floor, and Neurological Conditions in NYC

Dr. Barber treats a broad range of musculoskeletal, pelvic floor, and neurological conditions at his Upper West Side NYC clinic. This includes back pain, sciatica, neck and shoulder pain, SI joint pain, hip pain, TMJ dysfunction, herniated disc pain, tinnitus, trigeminal neuralgia, and complex pelvic floor conditions in males and females, such as pudendal neuralgia, chronic pelvic pain syndrome (CPPS), vulvodynia, vaginismus, interstitial cystitis, and chronic prostatitis. If your condition is not listed, a free discovery call can help determine whether Dr. Barber's approach is right for you.

Dry needling is a subset of acupuncture. Both use the same type of needle and often target the same tissues. The difference is in scope and how the treatment is applied.

Dry needling focuses on local tissue. It targets trigger points, muscle tension, and specific areas of dysfunction to reduce pain and improve movement. The needles are often not retained, meaning they are inserted, worked, and then removed rather than left in place.

Acupuncture is the broader system that dry needling comes from. It includes the same local tissue work, but also allows for treatment strategies that account for how the nervous system is functioning more globally. In many cases, needles may be retained for a period of time depending on the clinical goal.

In my clinic on the Upper West Side of NYC, these are not treated as separate methods. I use an integrated, neuro-functional approach, combining precise anatomical targeting with a broader treatment strategy based on the nervous system and the patient’s presentation.

Yes. Pelvic floor dry needling is one of Dr. Barber's flagship specialties and among the most advanced services available at any pelvic floor clinic in NYC. It involves releasing trigger points and hypertonic (overly tight) muscle bands within the pelvic floor that contribute to conditions like CPPS, pudendal neuralgia, levator ani syndrome, non-relaxing pelvic floor, pain with intercourse, urinary urgency and frequency, and postpartum pelvic pain. Treatment is performed externally, without the need for an internal exam or techniques, and is adjusted based on each patient’s diagnosis and comfort level.

Yes,  pudendal neuralgia is among the most complex and underdiagnosed conditions treated at this clinic. The pudendal nerve governs sensation throughout the perineum, genitals, and rectum, and when irritated or entrapped, it can cause severe burning, stabbing, or pressure pain that worsens with sitting. Dr. Barber uses neuro-functional acupuncture with targeted electro-stimulation to calm the sensitized nerve, combined with dry needling to release the surrounding musculature contributing to entrapment. Patients from across NYC,  including Midtown, Chelsea, and the Upper West Side, seek this specialized care after standard treatments have failed.

Dr. Barber treats non-somatic tinnitus,  the type not caused by a structural ear problem,  using neuro-functional acupuncture. This approach targets the neurological pathways involved in auditory processing and reduces the central nervous system hypersensitivity that perpetuates the ringing or buzzing. Research supports the use of acupuncture to modulate neural activity in the auditory cortex and reduce tinnitus severity. If you've been evaluated by an ENT and no structural cause has been found, this clinic offers one of the few specialized tinnitus treatment approaches in Manhattan.

Sciatica,  pain that radiates from the lower back through the glute and down the leg, is frequently treated with great success using orthopedic acupuncture and dry needling. Dr. Barber maps the myofascial kinetic chains contributing to nerve compression or irritation, including the piriformis, deep hip rotators, and lumbar stabilizers. For herniated disc pain, the treatment focuses on reducing local inflammation, releasing the surrounding musculature that loads the disc, and modulating the pain signals via neuro-functional acupuncture. This is a non-surgical, evidence-based alternative for patients seeking acupuncture for back pain and sciatica in NYC.

TMD (temporomandibular joint dysfunction) is a painful condition involving the jaw joint, surrounding muscles, and often co-occurring headaches. Acupuncture and dry needling are highly effective for TMJ pain by releasing the masseter, temporalis, and pterygoid muscles and associated trigger points that generate referred pain into the face, head, and neck. At this Upper West Side clinic, Dr. Barber also addresses the cervical spine muscles and upper trapezius patterns that frequently contribute to jaw dysfunction and neurogenic headaches. Many NYC patients who have not responded to night guards or dental approaches find significant relief through this targeted TMJ acupuncture protocol.

Yes. Shoulder pain and upper back or scapula pain are among the most common orthopedic presentations at this Manhattan clinic. Dr. Barber uses dry needling to deactivate trigger points in the rotator cuff muscles, rhomboids, levator scapulae, and surrounding tissue, while orthopedic acupuncture addresses the neurological drivers of chronic shoulder dysfunction. Whether the cause is a rotator cuff strain, postural overload, or a referred pain pattern from the cervical spine, treatment is precisely mapped and targeted, not generic. This approach is particularly effective for patients who have tried massage or PT without lasting improvement.

For orthopedic conditions like back pain, neck pain, sciatica, or shoulder pain, no prior referral is needed. For pelvic floor dry needling, Dr. Barber requires an active diagnosis from a qualified medical provider or pelvic floor physical therapist before proceeding. This is because pelvic floor dry needling works best as part of a broader, coordinated care plan,  not in isolation. Having a diagnosis ensures the technique is applied safely, appropriately, and in conjunction with whatever other interventions are part of your overall treatment strategy.
